Individual Registration

On-Line registration is a three step process:

1. "Submit" player information on this page.
2. "Print" a copy of your information and complete the Waiver Form on the Player Confirmation page.
3. Make check payable to Dream Sports Lacrosse. Mail signed Registration Form, Waiver Form and Payment to:
